Job Description

Position Overview

This Community Health Center is actively seeking a passionate and motivated Family Medicine Physician. Since the 1970s, our center has been the primary care medical home for thousands of Chicagoans – including some of the city's most vulnerable populations – by providing quality, accessible, and culturally sensitive primary healthcare. As a Family Medicine Physician, you will manage patient care in collaboration with a diverse, multi-disciplinary team and help us continue our mission to promote health, prevent disease, and deliver comprehensive care to underserved communities.

Key Responsibilities

  • Provide comprehensive and high-quality primary care services to patients in the office, hospital, or other community locations as assigned.
  • Serve as a consultant to clinical management regarding clinical goals and services.
  • Manage patient care in collaboration with other clinical staff and members of the care team.
  • Leverage industry knowledge and best practices to promote the mission and vision of the Medical Department and the organization.
  • Support culture development and management efforts.

Required Qualifications

  • Education: M.D. Degree or D.O. degree from an accredited school of allopathic or osteopathic medicine
  • Qualifications: Board-certified or Board-eligible in Family Practice
  • Current and valid Illinois license as a physician and surgeon
  • Current and valid DEA Controlled Substance Registration Certificate
  • Eligible for or maintaining hospital privileges at affiliated hospitals
  • Bilingual ability may be required based on individual department needs
  • Demonstrated ability to collaborate effectively in a team setting
  • Ability to maintain effective and professional relationships with patients and care team members
  • Comfort working as part of a multi-disciplinary team
  • Ability to observe, assess, and record patient symptoms, reactions, and progress
  • Knowledge of legal and ethical standards for the delivery of medical care
  • Familiarity with relevant drugs, non-pharmaceutical aids, and the ability to prescribe and instruct patients on correct usage
  • Knowledge of accreditation and certification requirements
  • Understanding of current principles and procedures for medical evaluation, diagnosis, and treatment
  • Experience working with a diverse population base
  • Ability to appropriately handle confrontation and conflict
  • Capability to work independently and seek consultation when necessary
  • Proven ability to follow organizational and departmental processes and procedures
  • Ability to work in a high stress, fast-paced, high-volume environment

  • Schedule: Monday - Friday 9:00am - 5:00pm; includes one extended shift (Noon - 8:00pm once per week) with a Saturday rotation every 4 weeks (8:00am - 1:00pm)
