Under the supervision of the Associate Director for the Office of Community Health and Quality Improvement, the Health Strategy Coordinator is responsible for 1) implementing and maintaining the agency’s performance management system, which is informed by the strategic plan, and tracking progress in real time. Reporting progress to the leadership team monthly and meeting with program supervisors to ensure the agency is meeting all goals on time, 2) connecting the goals and objectives from the Community Health Improvement Plan (CHIP) to Tulsa Health Department (THD) agency programs, 3) assisting with Quality Improvement (QI) projects, 4) assisting in the accreditation process by following through with assigned narratives, proof reading, creating cover letters, and tracking completion rates for each domain. Reporting progress and/or challenges to the Associate Director, and 5) grant writing for existing and new programs, with special emphasis on identifying resources to support the CHIP. Responding to requests for letters of support. Organizing all grant activity and reporting to the Associate Director as requested.The following functions represent most of the duties performed by the position but is not meant to be all-inclusive or prevent other duties from being assigned when necessary.

ESSENTIAL JOB FUNCTIONS:

  • Understand the agency’s strategic plan components and ensure implementation by entering data in the performance management system, tracking progress, and generating reports monthly as requested by the Associate Directors. Use data to guide decision making on where resources should be allocated, identify unmet community needs, and/or inform a QI project.
  • Supports the CHIP Project Manager by aligning CHIP goals and objectives with internal programs and ensures employees are connected to the CHIP work by attending quarterly and workgroup meetings.
  • Assists QI Manager on identified projects.
  • Assists accreditation efforts by following up with requested narratives and examples from agency offices. Proofreads collected information, prepares cover letter as needed, and uploads data and information into the appropriate SharePoint folder. Track accreditation progress in a cloud-based platform. Reports progress to Associate Director weekly.
  • Researches, identifies and secures grant and/or sustainable funding opportunities from Federal, State, local, and private foundations to support the THD mission and strategic plan. Writes letters of support, proposals, and grant applications. Build relationships with community partners through active participation in meetings, workshops, and seminars.
  • Work with the Health Data and Evaluation Department to research, organize, and interpret data on social and economic factors affecting the health of the local community.
  • Generates reports for the Board of Health monthly meetings.
  • Maintain an organized library (excel summary worksheet and folder per grant) of grants, both funded and unfunded, for the purpose of research and the potential to resubmit. Store in SharePoint allowing Associate Director of Community Health and Quality Improvement access.
  • May attend appropriate conferences, workshops, and other training events to enhance skills in program development, communication, and community health improvement.
  • This position must adhere to THD’s chain of command and management structure.
  • Perform other duties as assigned by the Associate Director and assist with events.
  • Complete required training in support of the duties and responsibilities of this position.
  • Other duties as assigned including those to fulfill activities in support of public health emergency operations.

Education:

Bachelor’s degree in social science, public health, health planning, business, or related field required. Master’s degree strongly preferred in project management, public health, health planning, business, or related field.

Experience:

  • Three to five years of professional-level experience in tracking intricate workflow outcomes and reporting associated outcomes.
  • Technical grant writing and research experience.
  • Community engagement experience.
  • Program development experience preferred.

Knowledge and Skills:

  • Proficient in Microsoft Word and MS Office applications.
  • Excellent oral and written communication skills.
  • Excellent research, problem solving, and proofreading skills.
  • Excellent human relations skills in maintaining effective and satisfactory working relationships with others with minimum direction.
  • Handles confidential work with tact and discretion.
  • Skill in dealing tactfully with the public.
  • Knowledge of gathering and analyzing data; and of planning, composing, and editing grants.
  • Must be highly motivated, detail oriented, and capable of working under demanding time constraints with minimal supervision.

INTERNAL AND EXTERNAL WORKING RELATIONSHIPS:

  • Internal contact with all THD programs in collecting data, conducting research, and developing grant proposals and projects.
  • External contact with community agencies, executives, private companies, foundations, and the Board of Health.

PHYSICAL EFFORT:

This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Lifts materials, boxes of records, and equipment weighing up to 25-30 pounds; stooping, bending, and squatting in retrieving files.

SUPERVISORY RESPONSIBILITY/ACCOUNTABILITY:

Direct Supervision: None.
Indirect Supervision: None.
Budget/Money/Material: Responsible for assigned equipment.
Reports to: Associate Director, Office of Community Health and Quality Improvement.

WORKING CONDITIONS:

  • Must be flexible to meet the needs of various schedules; hours may fluctuate based on the priority of deadlines and workload.
  • Managers usually work in an office environment, but the purpose of the project or meeting may sometimes take them to non-standard workplaces.
  • Managers work a standard workweek but may be required to work some evenings and weekends to meet grant deadlines or project milestones.

SPECIAL REQUIREMENTS:

  • Must maintain THD record confidentiality according to HIPAA guidelines.
  • Must possess ability and willingness to perform work-related travel.
