Job Description

Position Overview

Hennepin Healthcare is an integrated system featuring HCMC – a nationally recognized Level I Adult and Pediatric Trauma Center – along with an extensive clinic network and healthcare services including home care. The Home Health Program Manager will lead our Home Health Department, assuming overall responsibility for the oversight and management of both clinical and operational services. This role is based in MN-Minneapolis (MVNA Office) and offers an opportunity to make a meaningful impact in patient care while working in a diverse and equitable environment.

Key Responsibilities

  • Assumes overall responsibility for the delivery of care provided by the home health program staff.
  • Demonstrates a working knowledge of CMS regulations, Conditions of Participation, Joint Commission standards, and community standards for home health care.
  • Stays current with changes in rules, regulations, standards, and industry trends related to home health care (OASIS, 485 content and coding).
  • Oversees both clinical and non-clinical teams to ensure service excellence.
  • Maintains staff resources and ensures compliance with Hennepin Healthcare policies, Joint Commission, and CMS regulations.
  • Develops communication systems to sustain proactive referral flow management.
  • Collaborates with multidisciplinary care providers to enhance service delivery for home health program clients.
  • Facilitates professional growth by mentoring team leaders and clinical staff.
  • Leads the development of clinical policies and procedures and manages a robust quality improvement model.
  • Investigates patient complaints and utilizes data-driven models to enhance operational efficiency and quality improvement.
  • Serves as a subject matter expert, guiding team leaders and ensuring alignment of documented care plans (Oasis) with delivered care.
  • Conducts regular meetings with clinical staff and supports their educational and competency development.

Required Qualifications

  • Bachelor’s Degree in Nursing
  • Five (5) or more years of demonstrated leadership experience, preferably in home care including supervision and staff mentoring
  • OR an approved equivalent combination of education and experience
  • Possession of a valid license as a Registered Nurse

Preferred Qualifications

  • Master’s Degree in Nursing or leadership
  • Demonstrated management ability and in-depth knowledge of nursing practice, including rules and regulations pertaining to home care
