Job Description

Position Overview

Centene is transforming the health of our communities and is seeking a Care Coordinator II to support care management activities. In this role, you will assist care management teams in delivering high-quality services and ensuring continuity of care and member satisfaction.

Key Responsibilities

  • Provides outreach to members via phone or home visits, discussing care plans, next steps, and resources.
  • Coordinates care activities by collaborating with healthcare and community providers, caregivers, and members.
  • Supports member and provider inquiries related to care or service plans.
  • Communicates with care managers and practitioners to ensure continuity of care.
  • Assists in performing service assessments/screenings and documenting member care needs.
  • Maintains member records in accordance with state and regulatory requirements.
  • Identifies member needs and makes referrals to Care Managers, community organizations, or Disease Managers.
  • Provides education on available benefits and resources.
  • Performs other duties as assigned and complies with all policies and standards.

Required Qualifications

  • High School diploma or GED
  • 1–2 years of related experience
  • Must reside in Tampa or Orlando, FL
  • For Florida-Sunshine Health Plan, all interactions with members are telephonic

Preferred Qualifications

  • Availability for one 8-hour Saturday shift per month
  • Experience handling high call volume in a telephonic environment
  • Excellent Customer Service and Communication Skills
  • Experience with Health Risk Assessment
  • Background in Care Coordination, Case Management, Mental Health, or Education
