Registered Nurse RN Home Health

Position Overview

The RN Home Health collaborates with the RN Case Manager to develop and implement the home health plan of care by communicating with providers and various members of the care team. This role provides evidence-based, patient-centered care to individuals of all ages at their place of residence while utilizing critical thinking, effective decision-making, and a thorough understanding of OASIS documentation to optimize outcomes.

Key Responsibilities

• Collaborate with the RN Case Manager to develop and implement the home health care plan.
• Communicate with the patient’s provider and care team members (nursing, therapy, social work, home care aide, respiratory therapy, home medical equipment and home infusion services) to facilitate optimal outcomes.
• Provide patient-centered care using critical thinking and decision-making skills.
• Utilize thorough understanding of OASIS documentation and plan of care development to impact agency performance and value-based purchasing.
• Deliver appropriate care across the lifespan, from infancy through geriatrics.

Required Qualifications

• Completion of a nursing program.
• IL or MI RN Licensure (depending on work location).
• Current American Heart Association HealthCare Provider BLS before the start date.
• Excellent interpersonal and communication skills.
• Solid computer skills, including proficiency with Microsoft software.
• Strong analytical and problem-solving abilities with strong attention to detail.
• Must provide own transportation, maintain valid auto insurance, and hold a valid driver's license.

Preferred Qualifications

• BSN degree.
• Experience as an RN in a healthcare environment; Home Health OASIS experience is preferred.
• OASIS Certification.
• Basic proficiency with Electronic Medical Records and Microsoft applications.

• Hourly Position: Pay range of $36.59 - $53.17/hour (actual pay based on years of licensure).
