Health Home Care Coordinator (RN) Remote with Field Travel in King County

Position Overview
Molina Healthcare Services (HCS) works with members, providers, and multidisciplinary team members to assess, facilitate, plan, and coordinate an integrated delivery of care across the continuum, including behavioral health and long term care, for members with high need potential. This position supports the Washington State Plan. We are seeking a Registered Nurse with previous knowledge of behavioral health services, substance abuse, physical health/disease management, and long-term care. The candidate should have experience supporting social service needs, be familiar with community resources, and possess health promotion experience. The Health Home Care Coordinator must be comfortable with outreach calls to educate and enroll new potential members. Bilingual candidates are encouraged to apply.
Work Schedule: Monday-Friday, 8:00 AM to 5:00 PM PST. Remote position with field travel (60% within King County – Seattle, Bellevue, Kent, Federal Way, Kirkland, and Burien). Washington RN licensure required.
Key Responsibilities
- Conduct comprehensive assessments of members and determine eligibility for case management.
- Develop and implement collaborative case management plans involving members, caregivers, and healthcare professionals.
- Conduct face-to-face or home visits and monitor care plan effectiveness.
- Coordinate interdisciplinary care team meetings and facilitate outreach efforts.
- Utilize motivational interviewing and clinical guideposts to educate, support, and address barriers to care.
Required Qualifications
- Graduate from an Accredited School of Nursing (Bachelor's Degree preferred).
- 1-3 years experience in case management, disease management, or behavioral health settings.
- Active, unrestricted State Registered Nursing (RN) license in good standing (Washington RN licensure required).
- Valid driver's license with a good driving record and reliable transportation.
Preferred Qualifications
- Bachelor's Degree in Nursing.
- 3-5 years experience in case management, disease management, or behavioral health settings.
- Active, unrestricted Certified Case Manager (CCM) certification.
