Job Description

Position Overview

As a Clinical Pharmacist Manager in Illinois, you will be essential in our mission to provide better care and support to vulnerable Medicaid patients. You will work with a multidisciplinary team—including Community Health Workers, Care Coordinators, and Behavioral Health Therapists—to improve medication access, ensure adherence, and drive quality improvements through proactive outreach and analysis. Key Responsibilities

  • Provide telephonic, video, and SMS-based outreach, engagement, and education to vulnerable Medicaid patients in Illinois.
  • Collaborate with a multidisciplinary team to deliver wrap-around care management services supporting primary care practices.
  • Support patients and providers through transitions of care.
  • Deliver clinical services that ensure safe and efficient program operations.
  • Participate in quality improvement activities to ensure quality, cost-effectiveness, and return on investment.
  • Utilize analytics dashboards to proactively reach patients facing medication/equipment rejections or at high risk for acute care events.
  • Lead and mentor pharmacists and pharmacy technicians.

Required Qualifications

  • PharmD with advanced practice license in good standing in Illinois. Remote work is allowed in Illinois with a preference for candidates located in Chicago or central Illinois.
  • Completion of accredited PGY1 residency training in a related discipline.
  • 5+ years of experience in pharmacy practice, including direct patient care interventions in ambulatory settings serving vulnerable populations.
  • Experience in multi-disciplinary care management.
  • Positive referrals from previous colleagues.

Preferred Qualifications

  • Completion of PGY2 residency training in ambulatory care, geriatrics, pharmacotherapy, or internal medicine.
  • Strong analytical skills with the ability to navigate dashboards and registry data for proactive outreach, including addressing HEDIS care gaps.
  • Experience with medication management through collaborative practice agreements with primary care providers.
  • BPS Board Certification Preferred (preferably BCPS and/or BCACP).
  • Experience in serving vulnerable populations with skills in motivational interviewing, harm reduction, and behavioral health.
  • Leadership experience supporting pharmacists, pharmacy technicians, and/or other care team members.

  • Compensation (Salary Range): US Employees in Chicago: $98,000 - $151,000; US Employees outside Chicago: $95,000 - $147,000.
