Job Overview/Summary
The Ambulatory RN or LPN is responsible for delivering high-quality nursing care within an outpatient clinical setting. This role focuses on telephone triage, patient education, medication management, quality initiatives, and clinical support, ensuring efficient and patient-centered care in alignment with established practice guidelines.
Responsibilities
- Perform telephone triage according to established practice guidelines
- Provide comprehensive patient education and follow-up support
- Process and manage medication refills accurately and efficiently
- Participate in and support quality improvement initiatives
- Administer injections and other clinical procedures as necessary
- Document patient interactions, conversations, and actions taken accurately in the EMR
- Assist in developing and implementing applicable policies and procedures
Primary Skills
- Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license: in the State of Illinois
- Strong telephone triage skills
- Excellent oral and written communication skills
- Strong organizational and prioritization abilities
- Ability to accurately document patient care and clinical decisions
- American Heart Association CPR certification required
Secondary Skills (Good To Have)
- Experience with medical practice management systems
- Experience with EMR software, specifically Epic
- Proficiency in Microsoft Office Suite, particularly Word and Excel
Qualifications
- Current State of Illinois RN or LPN license: required
- Associate’s Degree in Nursing: required
- Bachelor’s Degree in Nursing (BSN): preferred; BSN must be achieved within 5 years of hire (if applicable)
- CPR certification: from the American Heart Association required
