Job Description

Position Overview

Allina Health is a not-for-profit health system dedicated to caring for individuals, families, and communities throughout Minnesota and western Wisconsin. In this role, you will provide holistic nursing care—including assessment, medication administration, patient education, and care coordination—within a dynamic care team. You will help screen clients for healthcare needs, develop client-focused case management plans, and ensure that care is safe, effective, and in line with current best practices.

Key Responsibilities

  • Perform vascular access and safe administration of medications, maintaining or inserting devices per best practice guidelines.
  • Educate patients and caregivers on appropriate home infusion and medication administration techniques.
  • Assess home environments for safety, ensure accurate documentation, and coordinate care in line with accreditation standards.
  • Collect venipuncture laboratory specimens and collaborate with patients and families to establish realistic, measurable outcomes.
  • Participate in quality improvement initiatives and evaluate personal nursing practice in accordance with professional standards.
  • Perform other duties as assigned.

Required Qualifications

  • Associate's or Vocational degree in Nursing
  • Minimum of 2 years RN experience

Preferred Qualifications

  • Bachelor's degree in a healthcare-related field (e.g., RN, PT, OT)
  • Case management experience
