In the Alignment Virtual Care Center, you will help patients access telehealth services 24/7—providing timely medical and social support to prevent unnecessary hospitalizations, health complications, and unmanaged disease progression. In this role, APCs will conduct virtual visits when patient triaging calls are escalated from the VCC RN and will also contribute to scheduled comprehensive visits. You will work within an interdisciplinary team including RNs, social workers, health coaches, and coordinators to support both clinical and social aspects of care.
Key Responsibilities
- Conduct virtual visits on Alignment members.
- Perform comprehensive assessments including past medical history, review of symptoms, physical examination, medication review, and cognitive/depression screenings.
- Provide patient education, emphasizing the importance of consistent monitoring of health needs and recommending community resources.
- Facilitate patient empowerment and quality of life by promoting educated, independent patient choice on all aspects of care when necessary.
- Match care desired with care given while providing true, informed consent.
- Identify diagnoses to be used in care management and active medical management in the furtherance of treatment.
- Formulate a list of current and past medical conditions using clinical knowledge, judgment, and the findings of your assessment.
- Communicate assessment findings to generate a post-visit summary note for the PCP, updating them and identifying potential gaps in care.
- Educate members on topics such as chronic conditions, self-care, sick-day plans, and medication compliance.
- Participate in weekly interdisciplinary team meetings to develop the most appropriate care plans based on patient needs.
- Comply with all HIPAA regulations and maintain the security of protected health information (PHI).
Required Qualifications
- Computer literacy/EMR proficiency
- At least one (1) year prior clinical experience
- Must have an active California license and be willing to obtain North Carolina and Nevada state licenses as a Nurse Practitioner/Physician Assistant
- Active Nurse Practitioner/Physician Assistant Board Certification
- Medicare Part D Prescriber (for Nurse Practitioners): Active RN License (Must, upon hire)
- Active NP License (Must, upon hire)
- Furnishing number (Must, upon hire)
- NPI Number (Must, upon start)
- DEA Number (Must, within 6 months of start)
- Board Certification: AANP, ANCC (Must, within 6 months of start)
- Valid BLS (Basic Life Support) (Must, upon start)
- Active PA License (Must, upon hire)
- Board Certification: NCCPA (Must, within 6 months of start)
- Knowledge of current clinical standards of care
- Ability to work independently
- Must be open to a flexible schedule as the position is active 24 hours a day, 7 days a week
Preferred Qualifications
- Valid ACLS (Advanced Cardiovascular Life Support)
- Experience in the care of older adult (geriatric) patients
- Home care experience
- Excellent administrative, organizational, and verbal skills
- Effective communication skills, especially with older adults
- Previous EPIC experience
