Job Description

Position Overview

This position is a unique role requiring excellent triaging and computer navigation skills. As a Transfer Center Nurse in a 24/7 department—with weekend and holiday rotations—you will field calls from outlying facilities regarding patients needing transfers to one of our multiple facilities. You will obtain reports, determine appropriate levels of care, coordinate with various medical professionals, and help arrange patient transport, ensuring that no delay affects the quality of care.

Key Responsibilities

  • Maintain patient flow throughout hospital units (emergency department, operating rooms, invasive recovery, cath labs, and nursing units) (20%).
  • Assess hospital admissions to ensure appropriate levels of care, medical necessity, and efficient resource use; includes telephonic triage and initiating clinical pathways (20%).
  • Plan and coordinate safe transfers for inpatients, working closely with nursing units to optimize patient placement (18%).
  • Collaborate with nursing administration to ensure resource allocation meets organizational needs (15%).
  • Continuously evaluate patients to identify changes in care requirements and guarantee optimal placement (9%).
  • Provide superior customer service to patients, families, and healthcare partners (5%).
  • Educate and support staff, including interns, residents, and attending physicians (4%).
  • Offer guidance on policies, crisis management, staffing, and departmental procedures (5%).
  • Identify opportunities for process improvement and deliver ongoing feedback (2%).
  • Act as a liaison among central Ohio hospitals to ensure timely and accurate patient placement based on bed availability (2%).

Required Qualifications

  • RN - Registered Nurse - Ohio Board of Nursing
  • 3-5 years experience in emergency department/critical care
  • Previous leadership roles (charge nurse, preceptor)
  • Excellent verbal and written communication, problem-solving, and organizational skills
  • Proficient computer skills (CareConnect) and strong typing skills
  • Experience in clinical pathway development and quality improvement initiatives
  • Work Shift: Day (Scheduled Weekly Hours: 36)
