Physician (MD) - Senior Care Rounding Specialist

Position Overview

Twin Cities Physicians is seeking a Mobile Primary Care Physician – Aging Adult Population to deliver relationship‐based care in diverse senior settings, from assisted living to memory care. Compensation: $225,000 – $250,000 annually and Location: Greater Twin Cities Metro Area. Twin Cities Physicians is changing the way seniors receive healthcare by providing proactive, community‐centered care directly in their living environments. Key Responsibilities

• Round at assigned senior living communities to deliver primary care services
• Provide medical oversight, diagnostics, and treatment planning
• Engage in chronic care management and reduce unnecessary ER visits
• Act as a clinical leader within an interdisciplinary support team
• Ensure thorough documentation and follow-up through EMR

Required Qualifications

• Active Minnesota MD or DO license
• Experience in geriatric medicine, internal medicine, or primary care
• Ability to work independently while collaborating with facility teams
• Valid driver’s license and reliable transportation
