Position Overview
Be a part of a world-class academic healthcare system, UChicago Medicine Care Network, as a Social Worker at our Kenwood clinic. You will work alongside our Family Medicine, Internal Medicine, and Senior Care team in a 100% onsite role based in the greater Chicagoland area. Our Kenwood location offers exceptional primary care for adults of all ages, featuring integrated services and specialized care. As a Social Worker, you will be a member of a multidisciplinary team providing consultation, assessment, and interventions for patients and families in need of emotional, social, or psychological support.

- Providing administrative coverage when requested
- Attending and participating in staff and other meetings as requested

- Serving on committees as assigned
- Supporting the philosophy and objectives of patient services as outlined in the annual ambulatory operating plan
- Demonstrating awareness of and compliance with organizational policies and procedures
- Complying with licensure, regulatory, and accrediting agency standards
- Meeting legal requirements as a mandated reporter, including reporting to agencies such as DCFS
- Adhering to all organizational policies and procedures
- Assessing personal needs for professional growth and seeking appropriate development resources
- Attending continuing education programs as an attendee or presenter
- Belonging to professional and/or specialty organizations
- Promoting Social Work within the hospital through events such as Social Work month, in-services, and lectures
Required Qualifications
- Bachelor’s degree in Social Work from a School of Social Work accredited by the Council on Social Work Education
- Knowledge of the principles of growth and development over the lifespan
- At least one year of clinical social work experience in a healthcare setting
- Current state of Illinois Social Work License (LSW) or eligibility for licensure, with the requirement to secure the license within the probationary period
