Position Overview

Home Healthcare Certified Nursing Assistant (CNA) in Rockford, IL and surrounding areas. Key Responsibilities

• Care for seniors and individuals with illness, injury, or chronic disease in their homes.
• Assist with daily activities such as bathing, toileting, dressing, grooming, hygiene, nail/skin care, eating, and nutritional intake.
• Help with ambulation, transfers, and range of motion exercises.
• Check vital signs, measure intake/output, monitor blood glucose, and document any changes.
• Collaborate with Home Health nurses and aides to ensure patient care plans are followed.
• Observe patients and promptly notify supervisors of issues requiring changes in care, medication, or medical equipment.

Required Qualifications

• High school diploma (or equivalent) and active Nursing Assistant Certification in Illinois
• CPR certification
• Valid driver’s license, auto insurance, and reliable transportation
• Compassionate nature, strong communication skills, and the ability to lift up to 50 lbs.
