Physician (MD) – Senior Care Rounding Specialist

Position Overview

Twin Cities Physicians is changing the way seniors receive healthcare by delivering relationship-based care directly to their living environments.
We are seeking a Mobile Primary Care Physician – Aging Adult Population (MD/DO) to provide primary care services at assigned senior living communities throughout the Greater Twin Cities Metro Area.Compensation: $225,000 – $250,000 annually.

Key Responsibilities

• Round at assigned senior living communities to deliver primary care services
• Provide medical oversight, diagnostics, and treatment planning
• Engage in chronic care management to reduce unnecessary ER visits
• Act as a clinical leader within an interdisciplinary support team
• Ensure thorough documentation and follow-up through EMR

Required Qualifications

• Active Minnesota MD or DO license
• Experience in geriatric medicine, internal medicine, or primary care
• Ability to work independently while collaborating with facility teams
• Valid driver’s license and reliable transportation
