Medical Transition Specialist

Position Overview

Patient Engagement Advisors (PEA) is seeking a full-time Transition Specialist to work onsite at AdventHealth Tampa. Schedule: Two shifts available; Monday – Friday 8:00 AM – 4:30 PM or 8:30 AM – 5:00 PM. As a Transition Specialist, your role is to set patients up for success as they transition out of the hospital into their next stage of care. At PEA, we are changing the way healthcare is delivered – one patient engagement at a time.

Key Responsibilities

• Schedule patient’s follow-up appointments
• Schedule patient’s transportation as needed
• Deliver discharge prescription medications to patient’s bedside
• Assist patients with medication affordability
• Coordinate home meal delivery service for patients
• Connect patients to community resources as needed

Required Qualifications

• High School Education or equivalent required
• 6 months pharmacy technician (retail preferred) OR 1-year experience as a medical assistant, patient care technician, patient care coordinator, clinic front desk associate, nursing assistant, home health aide, or in another healthcare setting
• Other related healthcare, hospital, or medical experience will be considered
• Ability to report to work onsite as scheduled; must be able to stand, walk, and climb stairs for 8–10 hours daily
• Ability to receive required vaccinations (as required by PEA or hospital) unless approved for exemption
