Position Overview
PCC Community Wellness Center is seeking a dedicated and experienced Part-Time Clinical Pharmacist to lead and support pharmacist-driven interventions within a multi-site Community Health Center. This role focuses on serving as a primary care provider liaison and improving hypertension (HTN) management and medication safety through direct patient care, remote monitoring, and collaborative quality improvement efforts. This is a GRANT-FUNDED POSITION through December 2026. Continuation of employment beyond the grant period is contingent upon additional funding. FLSA Status: Non-Exempt. Key Responsibilities
- Support management of PCC’s Hypertension Registry to prioritize patient outreach and treatment.
- Conduct Comprehensive Medication Reviews (CMRs) to optimize therapeutic outcomes.
- Identify, implement, and recommend medication changes based on clinical guidelines and patient-specific factors.
- Provide patients with clear, personalized instructions on medication use and adherence.
- Monitor and address medication side effects, ensuring patient safety and comfort.
- Attend and actively contribute to Live Healthy Chicago (LHC) Pharmacist Subcommittee meetings.
- Partner with the LHC Centralized Quality Improvement (QI) Team on HTN-related projects.
- Serve as a liaison to other healthcare providers to ensure continuity of care and collaborative decision-making.
- Educate patients, caregivers, and healthcare professionals on medication safety, proper usage, and potential interactions.
- Participate in performance improvement initiatives and provide consultative support to clinical teams.
Required Qualifications
- Doctor of Pharmacy (PharmD) degree from an accredited institution.
- Active pharmacist license in Illinois.
- Minimum of 2 years of clinical experience, preferably in ambulatory care or chronic disease management.
- Experience with remote patient monitoring and electronic health records (EHRs) is a plus.
- Strong communication, organizational, and analytical skills.
- Passion for community health and reducing health disparities.
