Assures that a complete clinical data set is accessible and available to all users. Responds to customer needs for clinical information in either electronic or paper format and works with Source corp in releasing the medical information for our internal and external customers.
Key Responsibilities
- Receives and reviews all requests for medical records for validity and verifies authorizations (non-billables only) including phone and fax.
- Processes all non-billable requests for release of information forwarded by Source corp.
- Obtains Medical Record numbers from “INVISION” for all non-billable requests and pulls medical records for each request.
- Retrieves data or records from various systems upon request of provider or outside reviewers as needed.
- Forwards records to Source corp for copying and mailing to the requester.
- Copies the records for non-billable requests, as needed.
- Assists the Supervisor with outpatient exception report and other reports as required.
- Provides phone support to the department and assists with inquiries.
- Reviews data in medical records or in electronic form for patient care, research, third-party inquiries, audits, and accreditation visits.
- Ensures that clinical documentation supports the diagnosis and treatment, is complete per hospital regulations, and supports billing for services rendered.
- Abstracts data as needed for patient care, follow-up research, and third-party reviews.
- Files Operative, Pathology, and other clinical reports.
- Enhances skill sets through continuing education in clinical documentation and release of information guidelines.
Required Qualifications
- High school diploma; some college coursework preferred.
- Three years of experience in hospital Medical Records or a comparable clinical setting.
- Knowledge of medical terminology.
- Strong PC skills, data entry, and database systems experience.
- Strong written and oral communication skills.
- Strong interpersonal skills and demonstrated customer service.
- Must be able to work independently with demonstrated leadership skills.

- HIM: 5 years (Required)
