Job Description

Position Overview

The Mobile Health Nurse is a registered or licensed nurse who is key to providing support for our mobile health providers and ensuring safe care for our patients. This role is vital in strengthening operational quality and improving clinical patient outcomes. The nurse assists in the coordination of patients’ healthcare needs while serving as a teacher, advocate, and navigator within the mobile health team. The Mobile Health Nurse reports to Director of Nursing and Chief Clinical Officer of Operations and Providers.

Key Responsibilities

  • Provides clinical and administrative support for providers via nurse visits.
    • Serving as a liaison between providers and their patients.
    • Communicating abnormal lab results and treatment plans per provider and clinic protocols.
  • Completes nursing tasks including checking INR with Coumadin titration, BP/BG checks, medication titration, transfer of scripts, patient education on chronic disease management, medication administration, pill organizer assistance, triage, pregnancy tests/counseling, and HIV rapid tests with result communication.
  • Executes nursing care under provider direction, such as IV infusions, medication injections, wound care and dressing changes, hygiene assistance, and podiatry care.
  • Collaborates with the care team to coordinate patient care, referrals, DME orders, care management, and hospital/ER follow-up.
  • Meets with patients for medication refills following clinic nursing protocol.
  • Directs, in conjunction with the provider, the assessment of acutely ill patients presenting to the mobile health team.
  • Uses assessment and triage to support patients in accessing the level of care they require.
  • Completes medication reconciliation and new patient intakes.
  • Conducts patient interviews to review medical history, active medications, chief concerns, and performs basic physical assessments. Reviews SBIRT, GAD/PHQ9, and mental health status exams, including managing PPD testing in required shelters.
  • Performs motivational interviewing/education for MAT/psych medication management and assists with assessing side effects after initiating new medications.
  • Carries out Medical Respite Rounding by scheduling and evaluating patients, writing care notes in EPIC, and following up with patients not present on-site.
  • Provides wound care, O2 weaning, and various forms of medication administration (direct oral, acute needs, and IM).
  • Conducts medication reconciliation, refilling of meds, and patient education on diet, proper medicine administration, physical activity, and diabetes management.
  • Assesses clinical status (triage), identifies decompensation, and communicates necessary changes in the care plan.
  • Updates the Respite Rounding List as needed.
  • Actively participates in care team meetings, monthly nurse meetings, clinic-wide quarterly meetings, and other meetings as directed by the Director of Nursing.
  • Documents all activities appropriately in the Electronic Medical Record.
  • Supports new initiatives within the Mobile Health department, including clinical care and the development of policies and procedures.

Required Qualifications

  • Strong patient assessment skills with the ability to triage effectively.
  • Effective communication skills, ensuring patient confidentiality and cultural sensitivity.
  • Flexibility to adapt to changing conditions and work schedules.
  • Good physical stamina with the ability to stand, move, and assist patients as well as handle equipment.
  • Excellent interpersonal skills to work with diverse ages, cultures, and temperaments, treating all with kindness and professionalism.
  • Commitment to personal integrity through punctuality, honesty, attention to detail, and a willingness to learn.
  • Ability to work independently, take initiative, and prioritize tasks in line with the team’s mission.
  • Clear and concise written and verbal communication skills.
  • RN licensure in the state of Illinois
  • CPR certification

Preferred Qualifications

  • Clinical experience in a clinic, acute care, or triage setting is preferred; new graduates with an interest in community health are encouraged to apply.
  • Knowledge of Spanish is helpful.
