Job Description

About the position

The Oncology Nurse Navigator at UChicago Medicine plays a crucial role in providing high-quality care to patients diagnosed with cancer. This position involves coordinating patient care throughout the disease process, acting as a clinical liaison among various medical services and departments, and ensuring that patients and their families receive comprehensive support and education regarding their treatment and care plans.

Responsibilities

  • Assess clinical, emotional, spiritual, psychosocial, financial, and other patient needs.
  • Facilitate resolution to patient needs and monitor follow-up.
  • Educate and reinforce education with patients and families on disease, treatments, side effects, adverse reactions, and reportable signs/symptoms based on multidisciplinary care plan.
  • Coordinate discharge planning with the patient's family, social workers, home nursing staff, oncologists, and personal physicians.
  • Work with patients and family members to ensure discharge instructions are understood and followed.
  • Develop and/or coordinate development of patient education materials, clinical pathways, and quality studies.
  • Coordinate and/or participate in multidisciplinary consultations and/or tumor conferences.
  • Document recommendations made at multidisciplinary consultations and/or tumor conferences.

Requirements

  • Registered Nurse: with a current Illinois license.
  • Graduate: of a National League for Nursing accredited School of Nursing with BSN.
  • Experience: Minimum of two years hematology/oncology clinical nursing experience with demonstrated clinical competence in patient care, teaching, and management of patients with a cancer diagnosis.
  • Knowledge: about clinical aspects of oncology including protocol therapy and data management.
