Position Overview
The responsibilities of this role include delivering hands-on nursing care and assistance within the LPN scope, with a focus on resident safety, clinical decision-making, and cooperation with the healthcare team. This position is vital for fostering rehabilitation and improving the overall well-being of residents within a nurturing atmosphere.
Key Responsibilities
- Effectively manage unit leadership duties each day and be proactive in meeting the requirements of residents and their families as outlined in the LPN practice standards.
- Exercise nursing discretion and abilities in determining the most important care needs of residents and delivering excellent hands-on care. Employ nursing skills and clinical decision-making to prioritize and administer quality direct care, using expertise and judgment to discern the urgency and significance of residents' care requirements.
- Perform customary nursing interventions and administer medications as directed, closely monitoring for expected or unfavorable reactions.
- Coordinate and cooperate with colleagues in the care team to modify nursing care plans and communicate pertinent discoveries.
- Precisely monitor and document vital signs, treatments, and resident reactions.
- Manage, monitor, and appraise CNA duties to ensure a successful delivery of care that meets residents' needs and workforce proficiency.
Required Qualifications
- Hold a degree from a recognized nursing school.
- Possess an active and unblemished Licensed Practical Nurse (LPN) license in New York State.
- Devoted to offering caring and medically appropriate nursing support.
- Effective communicator with strong organizational and customer support competencies.
- Able to maintain high performance levels in stressful conditions and foster effective teamwork among staff.
- Committed to providing support for residents facing illnesses, injuries, disabilities, old age, or occasional difficulties.
- Adherence to CDC guidelines with Covid-19 vaccinations strongly recommended at the facility.
