Position Overview
Premier Home Health Care Services, Inc. is a leading established Home Health Care Agency offering an opportunity for a Care Coordination Specialist in Southside Chicago, IL. In this role, you will provide support to our Care Managers by scheduling assessments, maintaining ongoing communication with participants, updating records, and assisting in the development of Care Plans based on assessments.
Key Responsibilities
- Schedule all assessments for participants and document all communication.
- Call vendors to retrieve accurate contact information for participants.
- Serve as the main point of contact for assigned participants and participate in the care planning process with Care Managers.
- Perform data entry for all State systems and meet required timeframe measures.
- Assist in coordinating services for assigned participants.
- Handle telephone calls related to participant safety incidents and relay details to the Care Plan Specialists and Supervisor.
Required Qualifications
- Minimum High School Diploma (or equivalent).
- At least one year of experience in health or human services.
- Possess general knowledge of office procedures.
- Ability to use computerized systems including Word and Excel.
- Ability to follow instructions and work independently.
- Excellent customer service and good verbal and written communication skills.
- Ability to work well under pressure.
- Travel may be required; must have a valid driver’s license and a satisfactory driving record.
Preferred Qualifications
- Bi-lingual a plus.
