Your Responsibilities
- Develop and implement strategies to improve population health outcomes in collaboration with internal and external stakeholders.
- Analyze data and trends to identify areas for improvement and track progress towards population health goals.
- Lead and manage a team of population health professionals to ensure effective and efficient execution of initiatives.
- Communicate and build relationships with key stakeholders, including healthcare providers, community organizations, and government agencies.
- Ensure compliance with all regulatory requirements related to population health management.
- Stay updated on industry trends and best practices in population health management and incorporate them into the organization's strategies.
- Develop and manage a budget for population health initiatives, ensuring resources are allocated effectively.
- Advocate for population health initiatives within the organization and externally to promote awareness and support.
- Collaborate with cross-functional teams to integrate population health strategies into existing programs and services.
- Evaluate the success of population health initiatives and make recommendations for improvement.
- Develop and deliver presentations and trainings related to population health management.
- Maintain accurate and detailed records and reports on population health initiatives and outcomes.
- Continuously seek opportunities for innovation and improvement in population health management.
