Your Responsibilities

• Develop and implement strategies to improve population health outcomes in collaboration with internal and external stakeholders.
• Analyze data and trends to identify areas for improvement and track progress towards population health goals.
• Lead and manage a team of population health professionals to ensure effective and efficient execution of initiatives.
• Communicate and build relationships with key stakeholders, including healthcare providers, community organizations, and government agencies.
• Ensure compliance with all regulatory requirements related to population health management.
• Stay updated on industry trends and best practices in population health management and incorporate them into the organization's strategies.
• Develop and manage a budget for population health initiatives, ensuring resources are allocated effectively.
• Advocate for population health initiatives within the organization and externally to promote awareness and support.
• Collaborate with cross-functional teams to integrate population health strategies into existing programs and services.
• Evaluate the success of population health initiatives and make recommendations for improvement.
• Develop and deliver presentations and trainings related to population health management.
• Maintain accurate and detailed records and reports on population health initiatives and outcomes.
• Continuously seek opportunities for innovation and improvement in population health management.
