Position Overview

Compensation: $90,000.00 - $115,000.00

Key Responsibilities

• Perform patient start of care comprehensive assessments, evaluation, development, and implementation of plan of care.
• Complete OASIS assessments in a timely and accurate manner.
• Provide skilled nursing care in accordance with the plan of care and agency policies.
• Communicate effectively with staff, physicians, family members, and community resources to ensure continuity of patient care.
• Initiate appropriate referrals as indicated.
• Promote and maintain patient health through teaching and rehabilitative measures.
• Assist patients in learning effective self-care techniques.
• Perform additional duties as assigned.

Required Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• At least one year of clinical experience as an RN. (If less than one year, approval by the VP of Clinical is required.)
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
