Job Description
OVERVIEW:
The Nurse promotes and restores patients' health by completing the clinical nursing processes and patient care duties; collaborating with health care providers and multidisciplinary team members; providing physical and psychological support to patients and their care support team.
KEY RESPONSIBILITIES:
Direct Patient Care:
- Administer medications, vaccinations, injections (e.g., IM, SC) to patients.
- Counsel patients/parents on vaccine/injection safety, side effects, and schedule.
- Perform and assist with basic office procedures (e.g., Depo shots, wound care, ear irrigations, etc).
- Provide patient education (e.g. chronic disease management, medication use, etc.) during visits.
- Assist with informing patients of any test results, based on ordering provider directions.
- Assess urgency of patient symptoms and direct to ER, urgent care, or same-day appointments.
- Perform telephone or in-person triage and advise patients accordingly.
- Identify any abnormal findings and escalate to the provider, if necessary.
- Protects patients by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations.
- Conduct nurse visits when permissible to include blood pressure checks, remote patient monitoring, general health education, diabetes education, etc.
- Identify and isolate infectious patients per protocol, ensure all PPE is utilized as needed.
Administrative Responsibilities:
- Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; and participating in professional societies.
- Perform prior authorizations for medications ordered by providers.
- Management of patient medications and authorize refills as a delegate of the provider.
- Review and manage outstanding items in assigned clinical inboxes in the EMR.
- Following hospital and nursing division's philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations.
Care Coordination:
- Assist with resolving patient problems and needs by utilizing multidisciplinary team strategies.
- Development of care plans for chronic conditions (diabetes, hypertension, asthma, etc.);
- Promote patient independence by establishing care goals;
- Education for patients and their supportive network of family and friends;
- Coordinate with referral specialists, partnered institutions, or other community resources for optimal patient care.
- Participate in transitional care management (TCM) after hospital discharge.
- Performs other duties as assigned.
REQUIRED QUALIFICATIONS:
- Bachelor's Degree in Nursing
- 3-5 years of related experience
- Clinical Skills, Bedside Manner, Infection Control, Nursing Skills, Physiological Knowledge, Administering Medication, Medical Teamwork, Multi-tasking, Listening, Verbal Communication, Health Promotion and Maintenance
- Excellent clinical and nursing skills
- Experience working with electronic medical records
- Ability to execute infection control practices
- Excellent Bedside Manner and interpersonal skills to effectively and courteously work with patients, families and co-workers
- Ability to administer medication
PHYSICAL REQUIREMENTS AND WORK ENVIRONMENT:
- Ability to sit or stand for extended periods of time
- Ability to work for extended hours with a computer
- Ability to lift up to 50 lbs
