Care Coordinator - Private Care Coordination [1361]

Position Overview

This role provides advocacy, coordination, and continuity of evidence‐based care for individuals and families in need through a collaborative process of triage, assessment, planning, and communication. As a key member of a multidisciplinary treatment team, you will work to reduce hospital emergency department and psychiatric inpatient admissions for high utilizer populations.

Key Responsibilities

• Provide advocacy, coordination, and continuity of care to individuals and families.
• Participate in a multidisciplinary treatment team focused on reducing emergency department and inpatient psychiatric admissions.
• Document clinical activities and communicate effectively with managers, patients, service providers, and other stakeholders.
• Safely operate a company vehicle and transport clients to appointments.

Required Qualifications

• Bachelor's degree in a Human Services-related discipline.
• At least one year of experience in the mental health field.
• Proven ability to document clinical activities and communicate effectively, both verbally and in writing.
• Valid driving skills with experience in transporting clients.
