Position Overview
Job Title: LPN/Care Coordinator. Compensation: $22 - $33 per hour. For over 50 years, TCA Health— a Federally Qualified Health Center on Chicago’s far south side—has provided innovative, accessible, and quality care to underserved communities. As the Care Coordinator/LPN, you will be responsible for monitoring and coordinating population health management activities at one or more health center locations. You will work closely with high-risk patients and their families to facilitate timely access to appropriate care, promote preventative measures, reduce emergency room utilization, and ensure smooth transitions—including school-based care transitions.
Key Responsibilities
- Identify patients for care coordination via registries and MCO care gap lists.
- Conduct timely patient outreach/engagement via telephone, mailing, face-to-face, or electronically to schedule or coordinate appointments.
- Track, monitor, and schedule follow-up appointments for patients recently discharged from the hospital or ER in collaboration with the care team.
- Assist patients with care transitions by navigating healthcare services and linking them to community resources and support.
- Collaborate regularly with care team members to resolve outstanding patient care items such as referrals and follow-up paperwork.
- Assist with data collection and reporting for targeted clinical quality measures.
- Follow up on referrals, diagnostic results (labs, x-rays, etc.), and consult reports with patients and specialty providers.
- Provide patient education resources to support self-management needs.
- Serve as a resource to the care team on population data management, patient outreach, and report generation.
- Collaborate with nursing staff to identify and refer appropriate patients for complex care management.
- Monitor clinical quality measure outcomes and work with leadership to identify improvement opportunities.
- Maintain timely, accurate, and thorough documentation in the EHR regarding patient communications and care services.
- Ensure confidentiality of patient information in accordance with HIPAA guidelines.
- Log and track all patient outreach efforts and appointment scheduling processes.
- Provide backup support for care team activities as needed.
- Maintain professionalism while communicating with patients, staff, and community partners.
- Adhere to agency policies, procedures, and NCQA Patient Centered Medical Home standards, participating in quality improvement initiatives and chart audits.
- Attend meetings and trainings to meet departmental productivity and performance objectives.
Required Qualifications
- Must possess a valid LPN license in Illinois or equivalent from an accredited program.
- Prior experience as an LPN is strongly preferred; experience as an EMT, CNA, Medical Assistant, or community health worker is a plus.
- Experience working in a clinical, hospital, or healthcare agency setting is preferred.
- Strong understanding of medical terminology, insurance authorization processes (Medicaid/Medicare and commercial), and electronic health record systems.
- Excellent customer service, interpersonal, and writing skills.
- Proficiency in computer applications and electronic health records is strongly preferred.
- Must have current CPR/AED certification.
- Ability to work independently, adapt to change, and communicate effectively with diverse age groups.
- Flexible work schedule and willingness to travel to school-based clinics and other satellite sites as necessary.
