Position Overview
Join our team as a Community Health Worker at Humboldt Park Health. In this role you will serve as a resource for program participants by providing education, support, and home health assessments during home visits. You will build referral relationships with healthcare providers, social workers, and community-based service providers while scheduling patient appointments and maintaining accurate records. Key Responsibilities
- Serve as a resource for participants in the program.
- Develop referral relationships with healthcare providers, social workers, and other community-based service providers.
- Perform home visits to provide education, support, home health assessments, and to obtain and complete consents.
- Follow-up on provider referrals and assist families in addressing barriers to health care access.
- Schedule patient appointments.
- Exhibit strong organizational and planning skills.
- Provide outreach to the community.
- Assist in the development of resource materials and in the preparation and maintenance of accurate records and data.
- Complete reports to document activities and provide data to measure impact.
- Perform other administrative office duties and maintain department orderliness and cleanliness.
- Adhere to downtime procedures and established departmental policies, procedures, and objectives.
- Model the mission and core values of Humboldt Park Health.
- Enhance professional growth by engaging in educational programs, literature, in-service meetings, and workshops/seminars.
- Check hospital email daily and perform other related duties as assigned.
Required Qualifications
- Completion of the Community Health Worker program (to be completed within 1 year of hire).
- Ability to speak and write English to ensure safe and efficient performance of job duties; strong preference for proficiency in Spanish.
