Part-time In-home visit Nurse Practitioner or Physician Assistant (East Los Angeles, CA)

Position Overview

Homebase Medical is getting back to basics with a mission to optimize the quality of life for homebound older adults by enabling them to live independently and with dignity. Backed by SCAN’s 45+ years of expertise, Homebase Medical is expanding rapidly into the E Los Angeles, CA area. We are looking for experienced part‑time Advanced Practice Clinicians (Physician Assistant or Nurse Practitioner) to join our expanding house call practice and deliver quality wrap-around services.

Key Responsibilities

• Perform primary care in the home setting, Board and Care, and Assisted Living Facilities
• Drive to patient locations in your assigned geographic area
• Conduct an average of 7-10 patient visits per day (if working a full workday)
• Exhibit stellar bedside manner; communicate complex medical information with simplicity and kindness
• Provide health education and assist patients in illness prevention, health promotion, and adjustment to chronic health problems
• Provide referrals to appropriate medical resources
• Utilize our EMR system to document patient interactions accurately and timely
• Demonstrate knowledge of HCC coding
• Participate actively in an on‑call rotation
• Perform other related duties as assigned

Required Qualifications

• Graduate of an accredited Physician Assistant or Nurse Practitioner program
• Current, unrestricted PA or NP license to practice in California, including DEA license
• Minimum of 1 year experience in primary care, hospice, or urgent care/outpatient settings
• Experience in a home health, hospice or other home visit setting is highly desired
• At least 1 year of experience utilizing an EMR system with a working knowledge of HCC coding
• Strong computer skills, including MS Office and Outlook
• Excellent verbal and written communication skills
• Valid driver’s license, auto insurance, and reliable transportation
• Spanish language skills are strongly preferred
