Position Overview

A well-established medical practice is currently seeking a dedicated Nurse Practitioner or Physician Assistant to provide care at skilled nursing facilities located around Louisville, Kentucky. Open to New Graduates – Comprehensive Training Provided

In this role, the advanced practice provider will be responsible for delivering high-quality medical care to residents in a long-term care setting. The ideal candidate is proactive, compassionate, and eager to grow within a collaborative care model.

Key Responsibilities

  • Conduct thorough patient assessments including medical histories and physical exams
  • Develop and implement care plans in alignment with patient needs and physician directives
  • Perform clinical procedures such as wound care, immunizations, and medication administration
  • Document encounters accurately in patient records
  • Educate patients and families on treatment plans and wellness strategies
  • Support continuity of care through coordination with facility staff and other healthcare professionals
  • Maintain compliance with safety protocols and regulatory standards
  • Stay current with clinical knowledge through continued education and professional development

Required Qualifications

  • Licensed Nurse Practitioner or Physician Assistant in Kentucky
  • Strong clinical judgement, communication, and organizational skills
  • A team player with a commitment to providing patient-centered care
  • Interest or experience in skilled nursing or geriatric care is a plus
