Job Description
Position Overview
Become a part of our caring community and help us put health first. Humana Gold Plus Integrated is seeking Long-Term Services and Support (LTSS) Care Coordinators (Care Coach 1) in Cook County, Illinois. In this role you will assess and evaluate members' needs to achieve or maintain optimal wellness by guiding members and families towards appropriate resources. You will employ a variety of strategies to manage health issues, work independently with some guidance from established procedures and professional standards, and continuously monitor care plans to ensure progress toward desired outcomes.
Key Responsibilities
- Visit Medicaid members in their homes, Assisted Living Facilities (ALFs), Long-Term Care Facilities, and other care settings – with 75-90% local travel.
- Ensure members receive services in the least restrictive setting by assessing their care needs.
- Identify and resolve barriers that hinder effective care.
- Plan and implement interventions to meet individual care needs.
- Coordinate services, monitor, and evaluate case management plans against members' personal goals.
- Continuously monitor patient care through assessments, data review, and active care planning to ensure progress toward desired outcomes.
- Guide members and families towards resources by facilitating interactions with payer sources, providers, interdisciplinary teams, and other care partners.
Required Qualifications
- Must meet one of the following:
- Bachelor's degree in social sciences, social work, human services, or a related field; or
- Active, unrestricted Licensed Practical Nurse (LPN) in Illinois with at least one year of experience in comprehensive assessments and formal services for elderly individuals.
- Applicants must reside in Cook County, Illinois or within a 10-mile radius of Cook County in a bordering county.
- Minimum of one year of experience in health care and/or case management.
- Experience working with Medicare and Medicaid recipients, long-term care services, Home and Community-Based Services (HCBS), and/or managed care organizations.
- Intermediate to advanced computer skills with proficiency in Microsoft Word, Excel, and Outlook, along with experience using electronic medical records and other software applications.
- Exceptional communication and interpersonal skills to build rapport with internal and external customers and stakeholders.
- Ability to travel regionally for face-to-face meetings with members, families, community partners, and care teams.
- Willingness to undergo TB screening as part of Humana’s Tuberculosis (TB) screening program.
- Must possess a valid state driver's license and maintain personal vehicle liability insurance as required by Humana’s driver safety program.
Preferred Qualifications
- Active, unrestricted Licensed Professional Counselor (LPC) in Illinois.
- Active, unrestricted Licensed Social Worker (LSW) in Illinois.
- Bilingual or multilingual proficiency (e.g., English/Spanish, Arabic, Vietnamese, Amharic, Urdu or other) with the ability to speak, read, and write without limitations.
