Community Health Nurse

Position Overview

This position is based in Lakeland, FL and requires daily travel within the Polk County territory. It is M–Friday, 10am–7pm.
Compassionate Care Hospice, an Amedisys Company, is seeking a dedicated RN Admissions Nurse to join our exceptional team in Lakeland.

Final compensation may vary based on location, job-related knowledge, skills, and experience.

Key Responsibilities

• Coordinates admissions of hospice patients by making skilled assessments of patients’ initial physical, psychosocial, and spiritual needs.
• Coordinates the establishment of the initial plan of care with the attending physician, medical director, and the interdisciplinary team.
• Provides skilled interventions and instructions to patients and caregivers to provide optimum symptom control and patient comfort under the direction of the physician.
• Functions as a liaison between disciplines and facilities during the admission process.
• Develops, prepares, and maintains individualized patient care progress records with accuracy and timeliness in accordance with policies. Submits accurate documentation per care center policy and records pain/symptom management changes/outcomes as appropriate.
• Communicates significant findings, problems, or changes in condition or environment to the clinical manager/physician and other members of the hospice care team; obtains physician orders as required.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.

Required Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• 2+ years of clinical RN experience in acute healthcare settings – REQUIRED.
• Hospice experience is a huge plus.
• Current CPR certification.
