Position Overview
Location: Northeast Chicago, IL | Position Type: Full-Time (Includes After-Hours On-Call). At Synergy Home Care, we are dedicated to delivering exceptional in-home care services with compassion, professionalism, and heart. We are seeking a versatile and motivated Care Services Coordinator & Community Liaison who bridges care supervision and outreach. In this role, you will oversee client care management and quality while building strong referral relationships with hospitals, case managers, senior communities, and healthcare professionals. NOTE: Participation in after-hours on-call rotation is required.
Key Responsibilities
- Care Supervision & Field Operations
- Conduct regular home visits to supervise caregivers and ensure client satisfaction
- Provide training, mentorship, and on-the-spot coaching for caregivers
- Address and resolve client or caregiver issues professionally and promptly
- Support onboarding and skills assessment of new field staff
- Maintain documentation of visits, incidents, and quality checks
- Step in for scheduling needs or emergency coverage as needed
- Participate in after-hours on-call rotation to respond to urgent needs
- Community Marketing & Client Outreach
- Cultivate referral relationships with hospitals, discharge planners, physicians, and community partners
- Represent Synergy at community events, senior expos, and health fairs
- Deliver presentations and informational sessions on home care services
- Track and follow up on leads and referral activities
- Collaborate with leadership on local marketing strategies and outreach goals
Required Qualifications
- Background in home care, healthcare, or senior services strongly preferred
- CNA, LPN, or healthcare certification a plus
- Proven experience in supervision and/or marketing/outreach roles
- Excellent communication, organization, and problem-solving skills
- Self-driven, personable, and confident in both clinical and marketing settings
- Valid driver’s license and reliable transportation required
- Availability for after-hours on-call rotation is required
