Position Overview
Starting Point Nursing Services is seeking anRN Tele-Triage (Fully Remote)professional with a compact state RN licensure. Start Date: 1/20/25. This part‐time role has 3 positions available immediately.
Updated Available Schedules:
- Thursday, Friday, and Saturday: 6–10a & 4–8p Central (24 hours)
- Monday, Tuesday, Wednesday & Sunday: 11p–6a Central (26 hours)
- Thursday, Friday, Saturday: 11p–6a Central (19.5 hours)
Key Responsibilities
- Monitor assigned phone lines during service hours to ensure timely response for caller needs.
- Triage all symptom‐based calls and provide recommendations according to approved triage protocols.
- Assess patients’ severity of symptoms and refer to the appropriate setting or level of care.
- Document in real time all care-related recommendations and notify local operations of triage supports and outcomes.
- Initiate contact with acute care settings to coordinate services for clients in the ED/Hospital and aid in discharge planning.
- Coordinate medication orders with the assigned pharmacy and update MAR/QuickMar for proper administration.
- Manage prior authorizations ensuring timely documentation and submission to the assigned pharmacy for billing.
- Conduct comprehensive assessments and reassessments for persons served as assigned.
- Develop and update care plans in collaboration with operations, securing physician orders as needed.
- Communicate with beneficiaries via phone, text, online chat, or email.
- Ensure compliance with state, local, and payer requirements through diligent documentation reviews.
- Provide coaching and education regarding the health and well-being of persons served.
- Participate in performance improvement activities and maintain current clinical knowledge through internal/external training.
- Record actions in the Nurse’s Notes or Service Delivery Log accurately.
- Maintain a timesheet log for weekly productivity management submission to the supervisor.
- Notify each practice, physician, and/or managed care client of all patient encounters prior to office opening.
- Alert designated leadership of any operational concerns.
- Actively participate in required meetings and committees (e.g., IDT, HRC, Safety/Quality, and annual treatment plan meetings).
Required Qualifications
- Degree or certification from an accredited school of nursing.
- Current licensure as a Registered Nurse in the state of practice, in good standing; must have a compact state RN license.
- Clinical experience providing services and supports to individuals with intellectual and developmental disabilities, home health, or hospice patients.
- Minimum of two years’ experience in Home Health nursing.
Compensation
- Salary Range: USD $30.00 - $35.00 / Hour
