Position Overview

The Field Care Manager Nurse 2 assesses and evaluates members' needs to achieve and maintain optimal wellness. This role guides members and families toward appropriate resources for effective care and overall wellbeing.

Key Responsibilities

• Employ various strategies to manage physical, environmental, and psycho-social health issues.
• Identify and resolve barriers that hinder effective care.
• Continuously monitor patient care through assessments and evaluations to ensure progression toward desired outcomes.
• Develop individualized member care plans as needed.
• Interpret departmental and organizational strategies to inform decision-making and work methods.
• Make independent decisions in ambiguous situations while following established guidelines and procedures.

Required Qualifications

• Registered Nurse with at least 2 years of experience in home case/care management.
• Experience working with the adult population and knowledge of community health and social service agencies.
• Ability to travel within 30–40 miles to members' residences.
• Exceptional communication and interpersonal skills with the ability to quickly build rapport.
• Ability to work independently with minimal supervision.
• Proficiency in using various electronic information applications/software, including electronic medical records, Microsoft Word, Outlook, and Excel.
• Must commit to a full-time (minimum 40 hours) flexible work schedule.
• Participation in Humana's Driver Safety program: valid driver's license, car insurance, and personal vehicle liability insurance meeting required limits.
• Must have a dedicated home office with a locked door to ensure continuous privacy.
• High-speed DSL or Cable internet is required for optimal system performance.
• Role involves patient-facing responsibilities, including tuberculosis (TB) screening.
• Associates in the State of Florida require ACHA Level II Background clearance.

Preferred Qualifications

• BSN preferred.
• 3–5 years of in-home assessment and care coordination experience.
• Experience with health promotion, coaching, and wellness.
• Previous managed care experience.
• Bilingual in English and Spanish.
• Certification in Case Management.
• Motivational Interviewing Certification or equivalent knowledge.
