Position Overview

Deliver skilled nursing care in a home health setting focused on patients with complex cardiopulmonary and post-operative needs. This role requires clinical expertise in wound care, IV therapy, and managing high-risk diagnoses such as heart failure, COPD, myocardial infarction, pneumonia, and stroke. Work directly with patients to provide assessments, treatments, and support while documenting care using dedicated electronic systems.

Key Responsibilities

• Manage 6-8 skilled home visits per day within scheduled daytime hours (varied shifts available Monday through Friday).
• Perform comprehensive assessments including cardiopulmonary evaluations and wound care management (e.g., ABI compression dressings, wound vacs, suture and staple removal).
• Administer injections, lab draws, venipuncture, and PICC/Midline care.
• Document patient care thoroughly using BayCare electronic tools.
• Coordinate visits using provided laptop and mobile internet equipment.

Required Qualifications

• Minimum 1 year of skilled home health nursing experience (private duty nursing alone will not be accepted).
• OASIS training and proficiency required.
• Competency in IV skills, wound care, and medical/surgical assessments.
• Certification in BCLS (Basic Cardiac Life Support).
• Valid driver's license and reliable vehicle for home visits.
• COVID-19 vaccination or exemption on file; flu shot or declination with compliance to mask protocols.
