Position Overview
The Home Health Care Liaison serves as the primary connection between referral sources, patients, families, and the home health agency. This role facilitates seamless transitions from hospitals, clinics, or community settings to home-based care, ensuring timely referrals, excellent communication, and positive patient experiences. The liaison promotes agency services, builds strong relationships with referral partners, and supports census growth through effective outreach and coordination. The position is based in the Twin Cities Metro area with an office in Edina.
Key Responsibilities
- Communicate with clinical staff to ensure timely start-of-care visits.
- Provide patients and families with information about home health services, expectations, and care processes.
- Connect with referral sources to ensure processes and referrals run smoothly and efficiently.
- Develop and maintain strong relationships with referral sources including discharge planners, case managers, physicians, and social workers.
- Participate in on-site visits, presentations, and meetings to promote agency services and identify new referral opportunities.
- Serve as a liaison between referral sources, the home health clinical team, and administrative staff while ensuring prompt communication of patient needs and referral updates.
- Coordinate with intake, scheduling, nursing, and therapy teams for efficient transitions to home care.
- Maintain accurate records of referral activity, track trends, and report conversion rates and barriers to leadership.
- Ensure compliance with HIPAA and regulatory requirements during the referral process.
- Educate referral sources and community partners about agency services, admission criteria, and covered benefits (Medicare, Medicaid, private insurance).
- Provide ongoing support to patients and families to help them understand home health requirements and next steps.
Required Qualifications
- Bachelor’s degree in marketing, Social Work, or a related field is preferred but not required.
- Experience in home health, case management, discharge planning, or healthcare sales is highly preferred.
- Strong communication, relationship-building, and interpersonal skills.
- Knowledge of Medicare home health guidelines, admission criteria, and community resources.
- Ability to multitask effectively in a fast-paced environment with excellent organizational and documentation abilities.
- Capability to work autonomously while remaining connected to the home health team.
- Excellence in oral and written communication along with professional presentation and outreach skills.
