Job Description

Position Overview

Humana is seeking a Care Manager, Telephonic Nurse 2 to work in a telephonic environment across nationwide locations, with primary operations in Tampa, Florida. In this role, you will assess and evaluate members’ needs to help achieve or maintain an optimal state of wellness by guiding members and their families to the appropriate care resources. You will also determine the best course of action independently while aligning with departmental and organizational strategies.

Working Hours: 8:00am-7:00pm CST (core hours 8:00am-5:00pm CST) | Scheduled Weekly Hours: 40

Key Responsibilities

  • Employ a variety of strategies to manage members’ physical, environmental, and psycho-social health issues.
  • Identify and resolve barriers that hinder effective care.
  • Continuously monitor patient care through assessments and evaluations to ensure progress toward desired outcomes.
  • Develop and update member care plans when needed.
  • Follow established guidelines and procedures while making informed decisions independently.
  • Align work with the department and organizational strategies and operating objectives.

Required Qualifications

  • Bachelor’s degree
  • 3 - 5 years of clinical acute care experience
  • Licensed Registered Nurse (RN) in the appropriate state with no disciplinary action
  • Comprehensive knowledge of Microsoft Office applications: Word, Excel, and Outlook
  • Passion for continuously improving consumer experiences

Preferred Qualifications

  • Bi-Lingual (English/Spanish)
  • Certified Case Manager (CCM)
  • Experience with case management, discharge planning, and patient education for adult acute care
  • Managed care experience
  • Previous experience with Medicare & Medicaid recipients
  • Experience with electronic case note documentation and proficiency with multiple computer applications/systems
  • Experience with health promotion, coaching, and wellness
  • Knowledge of community health, social service agencies, and additional community resources
