Job Description
Position Overview
Be a part of a world-class academic healthcare system, UChicago Medicine, as a Staff Pharmacist in the Pharmacy Department. You will provide clinical and distributive pharmacy services for patients within your assigned area while actively participating in education, research, and quality assurance activities. Position details include: Job Type/FTE: Registry (0.2 FTE); Shift: 7am-8pm rotating; Unit/Department: Pharmacy/Specialty OP- Hyde Park; Work Location: Calumet City; CBA Code: Non-Union. The work schedule generally consists of Monday–Friday with every other weekend.
Key Responsibilities
- Ensure the accuracy and appropriateness of all medication orders by evaluating drug selection, dose, frequency, safety, dosage form, allergies, interactions, and disease state factors prior to dispensing.
- Evaluate patient information (age, weight, medical characteristics, and laboratory results) to ensure proper medication dispensing.
- Provide age-appropriate care by demonstrating knowledge of growth, development, and data interpretation related to patient status.
- Supervise pharmacy technicians in tasks such as dose preparation, order entry, STAT medication delivery, IV therapy coordination, stock replacement, and maintaining general cleanliness.
- Identify service requirements through interaction and consultation with patients, physicians, nurses, pharmacists, and technicians.
- Monitor proper drug utilization by ensuring compliance with drug use criteria, clinical pathways, and IV to PO conversion.
- Participate in drug utilization review and other quality assurance programs.
- Engage in the chemotherapy quality assurance program including preparation, monitoring, and dosage calculations for chemotherapeutic medications.
- Exercise professional judgment in evaluating the safety and efficacy of medication orders.
- Serve as the primary resource for drug information and educate patients regarding their medication therapy, including potential side effects and management.
- Ensure proper dosing based on pharmacokinetic principles and maintain serum drug level monitoring.
- Document clinical interventions that prevent or resolve drug-related problems and promote cost-effective therapy.
- Provide consultation for assigned patient populations and communicate recommendations to medical staff effectively.
- Maintain and enhance professional knowledge through continuous education, training, and literature review.
- Assist in the orientation and training of new personnel.
- Participate in ambulatory pharmacy dispensing activities, including dispensing, counseling, and clinical services in an outpatient setting.
- Perform other duties as requested by senior management.
Required Qualifications
- Graduate of an ACPE accredited School of Pharmacy
- Registered pharmacist in the state of Illinois
- Exceptional aptitude or knowledge may be considered in lieu of experience
- Knowledge of contemporary hospital/clinical practice and service
- Strong written and verbal communication skills
- Ability to interact constructively with supervisors, peers, support personnel, and students
- Understanding of general hospital operations and ability to integrate clinical, distributive, educational, and research activities
Preferred Qualifications
- Doctor of Pharmacy or another advanced degree
- Completion of an ASHP accredited residency
- Two years hospital pharmacy experience in an organized health care setting
