Job Description

Position Overview

We are looking for qualified Registered Nurses to join our fully remote 24-hour triage center. In this role, you will receive incoming calls for clients who need immediate medical attention and provide assessment, guidance, and proper disposition of care. You mustMUST LIVE WITHIN A 2 HOUR DRIVE TO THE WNY AREAand be able to work rotating (4) 10-hour shifts, including weekends with premium shift differential.Compensation:$68,000-$71,500 along with an excellent medical benefits package.

Key Responsibilities

  • Receive incoming calls from contracted clients.
  • Assess patients’ current health status and recommend the appropriate level of care based on clinical judgment, evidence-based guidelines, and protocols.
  • Develop plan(s) of nursing service tailored to the patients’ healthcare needs.
  • Demonstrate knowledge of medications and telephone communication skills with proper etiquette.
  • Make critical independent decisions and prioritize appropriately.
  • Document all calls and actions taken in the patient’s medical record.
  • Utilize technology as required to fulfill job responsibilities.
  • Communicate assessment outcomes to healthcare team members to ensure proper follow-up care.
  • Exhibit patience, courtesy, and flexibility while interacting with patients, physicians, and staff.
  • Adhere to ethical, legal/regulatory, and accreditation standards.
  • Take responsibility for your own education and professional growth.
  • Consult with the administrator for issues outside your scope of practice.
  • Maintain excellent attention to detail and multitask as needed.
  • Perform other duties as assigned.

Required Qualifications

  • New York State Registered Nurse Licensure Only.
  • Unrestricted New York State RN with a minimum of two years of experience as a clinical hospital nurse in emergency, critical care, or med/surg OR two years’ experience in RN telephone triage in a primary care setting.
  • Previous Chronic Care Management / Case Management experience required.
  • Previous Chronic Disease Management and Education experience required.
  • Current CPR/BLS certification required.
  • Current Infection Control Certificate required.
  • Proficiency with advanced computer technology and multiple computer applications.

Preferred Qualifications

  • AAS in Nursing, Bachelor’s Degree in Nursing, or related field (preferred).
  • Knowledge of OPWDD regulations (recommended).
