Saint Therese of Oxbow Lake is seeking an MDS Coordinator (RN) to join our dedicated team. Since 2006, we have ensured seniors enjoy a rewarding lifestyle at our vibrant Brooklyn Park campus, which features 142 independent living, assisted living, and memory care apartments, plus 64 long-term and transitional care suites. This role is part-time, typically working on Tuesday and Thursday each week. Location: Brooklyn Park, MN. Schedule: Part-time, Tuesday & Thursday.
Salary: $32,614 - $44,262 annually based on experience.
Key Responsibilities
- Oversee and complete MDS and care planning.
- Complete all documentation for RAI and MDS assessments and observations.
- Develop initial and temporary care plans.
- Attend resident care conferences.
- Manage and assign tasks to nursing staff on the assigned unit.
- Work in collaboration with Admissions staff and the nursing team.
- Attend discharge meetings as requested.
Required Qualifications
- RN license in the state of Minnesota is required.
- Must have MDS experience.
- Must have previous long-term care experience as a nurse.
- Proficient in computer software/EMR documentation.
Preferred Qualifications
- Demonstrated knowledge of State and Federal regulations, including the Resident Assessment Instrument.
- Demonstrated extensive knowledge of Case Mix.
- Demonstrated extensive knowledge of Medicare, HMO's, and Managed Care.
- Good written and verbal communication skills.
- Knowledge in ICD9/10 Diagnosis Coding.
- Flexible with time management and prioritization skills.
- Ability to maintain an effective working relationship with nursing staff and other departments.
- Familiarity with software such as Microsoft, ROX, Facility Software, and Excel.
- Ability to work with others in identifying and improving quality of care issues and solutions.
- High level of respect and commitment to working both independently and collaboratively.
