Position Overview

We are Reliance Home Health Caregivers – a home health care company dedicated to long-term 1-on-1 private duty nursing for disabled pediatric and adult patients on G-tubes, trachs, and vents. We are urgently seeking a compassionate nurse for immediate hire in the Chicagoland Area. Join a team that truly cares about its nurses, families, and patients.

Key Responsibilities

• Provide 1-on-1 nursing care for a 26-year-old female patient with G-Tube and Trach needs.
• Work scheduled shifts Monday through Wednesday, 7pm–7am in Chicago, IL 60641.
• Deliver exceptional in-home healthcare tailored to the patient’s individual needs.
• Collaborate with schedulers, case managers, and office staff to ensure coordinated care.
• Offer compassionate support to both the patient and their family.

Required Qualifications

• Possess a valid LPN or RN license in the state of Illinois.
• Minimum of 1 year of direct patient care experience as an RN or LPN.
• Valid CPR card.
• Strong commitment to high-quality, compassionate patient care.

Preferred Qualifications

• Pediatric nursing experience is a plus, but not required.
• Willingness to participate in training for pediatrics and home health skills.
