Position Overview

In thisremote role, you will serve as a Disease Management Nurse responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating members’ health and psychosocial needs to prevent complications and support optimal care.
Key Responsibilities
- Identify and execute interventions and overall care plans in collaboration with an interdisciplinary team including social workers, community health workers, care coordinators, and pharmacists.
- Evaluate the clinical appropriateness of care plans by integrating assessment findings with evidence-based guidelines and best practices.
- Promote member activation and engagement to ensure optimal self-management skills and health outcomes.
- Provide education and coaching on disease management by identifying new or worsening chronic conditions.
- Create and direct interventions for disease management pods, including guiding field-based initiatives by Community Health Workers.
- Identify opportunities to improve existing processes and enhance departmental activities.
- Participate in continuous professional development through education, professional publications, and networking.
- Comply with regulatory and quality standards including those from CMS, DMHC, DHS, and NCQA.
- Support SCAN’s vision, mission, and organizational initiatives related to member education, quality care, and community engagement.
- Perform other related duties as assigned.
Required Qualifications
- Current & Active CA RN License required
- BILINGUAL:Must be proficient in English/Spanish (proficiency test will be administered)
- 1-2 years of Disease Management experience required
- At least 1 year of direct patient care nursing experience
- Basic knowledge of NCQA standards, CMS, and DHCS regulations
Preferred Qualifications
- BSN degree preferred
- 3+ years of clinical experience with the geriatric population (Acute, Ambulatory care, SNF and/or LTC)
- 2+ years of case management experience in a medical group, IPA, and/or HMO setting
- Experience with Medicare/Medi-Cal in a managed care environment
- CCM Certification preferred
- Strong skills in Coaching and Motivational Interviewing with the ability to build effective rapport
