Job Description
Position Overview
The Internal Medicine Clinical Pharmacy Specialist provides advanced clinical pharmacy services to optimize medication use and improve patient outcomes for adult internal medicine/surgical patients within a large academic medical center. In this role, you will deliver both operational and clinical pharmacy services, including comprehensive medication management, therapeutic drug monitoring, and participation in patient care rounds alongside interdisciplinary teams. Location: Chicago, Illinois. Work Schedule: Full Time (7:00 AM - 3:30 PM). Compensation: $53.73 - $84.63 per hour.
Key Responsibilities
- Perform order verification, discharge counseling, and execute projects while educating other healthcare professionals.
- Evaluate and verify Epic physician orders for appropriateness including dosing, dosage form, drug interactions, and administration details.
- Clarify physicians' orders when needed through direct communication with prescribers.
- Provide patient-specific pharmacotherapy recommendations and drug information to medical and nursing staff.
- Document clinical interventions per departmental standards.
- Review pharmacy reports daily including pharmacokinetic monitoring and other necessary patient-specific reports.
- Conduct monthly inspections of nursing unit medication storage areas.
- Supervise pharmacy technician staff and coordinate workflows in medication preparation and delivery.
- Coordinate daily medication ordering, preparation, dispensing, and administration systems.
- Implement drug use policies and formulary decisions as set by the Pharmacy and Therapeutics committee.
- Participate in adverse drug reaction (ADR) and medication error reporting programs.
- Undertake clinical and operational activities including inventory optimization and process improvement projects.
- Precept pharmacy students and residents (PGY1 and PGY2) in clinical and operational areas.
- Evaluate medical and drug literature to compare and contrast therapeutic regimens.
- Implement medication safety initiatives and develop relevant projects as needed.
- Serve as a liaison with other departments for collaborative research activities.
- Monitor drug therapy relative to the overall needs of ambulatory care patients, collaborating with physicians, nurses, and other professionals.
- Actively participate in professional organizations and contribute to rotational staffing (weekday evenings 25%, every 4th weekend, and holidays).
Required Qualifications
- Bachelor of Science in Pharmacy.
- Illinois Pharmacist License.
- Minimum of four years of Clinical Pharmacy experience OR PGY 1 residency completed within the last seven years, including Clinical/Specialty Pharmacy experience.
- Knowledge in intravenous admixture services, intravenous drug therapy, and chemotherapy.
- Demonstrated ability to work effectively with others to ensure safe and effective drug therapy.
- Excellent problem solving skills with the ability to work independently and quickly resolve issues.
Preferred Qualifications
- BLS Certification; ACLS Certification (or must be obtained upon hire).
- Board Certification by the Board of Pharmaceutical Specialties.
- Doctor of Pharmacy degree and residency certificate.
- Extensive experience and strong knowledge in specialty drug therapies.
