Position Overview

This Care Coordinator II role supports care management activities to ensure that services are delivered by healthcare providers and partners, achieving care continuity and high member satisfaction. We are seeking a Care Coordinator II to join our team!
Key Responsibilities
- Provides outreach to members via phone or home visits to engage them and discuss the care/service plan, next steps, resources, and any questions or concerns related to recommended care and ongoing education.
- Coordinates care activities based on the care/service plan, working with healthcare and community providers, partners, and members/caregivers to accommodate changes or progress as needed.
- Serves as support for various member and provider inquiries, requests, or concerns related to the care/service plan.
- Communicates with care managers, practitioners, and others to facilitate member services and ensure continuity of care/service.
- May support performing service assessments/screenings for members and document the member’s care needs.
- Documents and maintains member records in accordance with state and regulatory requirements, distributing them to providers as needed.
- Follows standards of practice and policies compliant with contractual requirements and regulatory guidelines.
- Identifies member needs and makes referrals to Care Manager, community-based organizations, and Disease Manager.
- Provides education on benefits and available resources.
- Performs other duties as assigned and complies with all policies and standards.
Required Qualifications
- Must reside in Tampa or Orlando, FL.
- One 8-hour Saturday shift per month.
- Experience working in a high call volume, telephonic environment.
- Excellent customer service and communication skills.
- Experience in health risk assessment and care coordination.
- Background in case management, mental health, or education.
- High School diploma or GED.
- 1–2 years of related experience.
- License/Certification: For Florida-Sunshine Health Plan – all interactions with members are conducted telephonically.
