Job Description

Position Overview

Under direct supervision, maintains a comprehensive medical record consistent with all federal, state and company policies and procedures, and performs general secretarial duties to support their assigned interdisciplinary team(s).

Key Responsibilities

  • Generates and electronically files appropriate patient related information for the medical record consistent with company policy and regulatory requirements.
  • Prepares mailings to physicians and ensures that all MD orders are received, signed and returned within 30 days of receipt, including hand delivery when necessary.
  • Regularly runs Missing Plans of Care and Orders reports and works to obtain missing information.
  • Requests medical records from various sources such as doctors’ offices, hospitals, facilities and/or family members.
  • Prints patient care plans, medication order sheets and demographic information from the electronic medical record for patients being transferred to other facilities and responds to record requests to facilitate treatment delivery.
  • Supports and attends weekly interdisciplinary team meetings and updates the EMR with documentation from these meetings.
  • Prepares all documentation for IDT members, collects documentation from clinical staff during meetings and enters the data into the EMR.
  • Prepares necessary paperwork for re-certifying patients for ongoing care per regulatory guidelines.
  • Reviews documentation to ensure quality and accuracy, maintains meeting attendance records, and prepares agendas to ensure all patients are discussed within regulatory guidelines.
  • Conducts regulatory audits of patient charts and reviews inactive patient records for timely closure.
  • Communicates with pharmacies regarding prescriptions and coordinates record releases in response to external requests.
  • Identifies patient needs and regulatory requirements associated with medical insurance.
  • Prints MTM assessment forms for daily review, obtains required signatures, and scans forms into the EMR weekly.
  • Provides administrative support by preparing daily reports and gathering data for administrative and statistical reports.
  • Responds to phone calls, emails, and mailings in a timely manner while providing coverage for other teams as needed.
  • Prepares for emergency events by maintaining accurate patient acuity scales in the EMR.
  • Attends all meetings designated by the supervisor and operates standard office equipment.
  • Prepares patient charts for review by the Department of Appeals and processes discharges, revocations, or transfers appropriately in the EMR.
  • May process and respond to requests for radiology images following HIPAA guidelines and privacy regulations.

Required Qualifications

  • HS Diploma, GED or equivalent.
  • 1 year’s customer service or administrative experience; additional education beyond high school may be substituted for experience.
  • General knowledge of office practices, procedures, and equipment; proficiency in business English, spelling, punctuation, grammar, and basic arithmetic.
  • Proficient in using office software applications such as word processing, spreadsheets, presentation packages, and database applications.
  • Strong oral and written communication skills with the ability to accurately follow instructions.
  • Skill in accurate alphabetical and numeric filing.
  • Able to multi-task and prioritize in a busy environment with strong time management skills.

Preferred Qualifications

  • Experience in the health care field.
  • Knowledge of medical terminology.

  • Compensation: $16.85 - $25.59 (Final salary offer will be based on the candidate’s qualifications, education, experience, and alignment with organizational needs.)
