Service Coordinator, Supervisor- Philadelphia, PA & Surrounding Areas (Hybrid)

Position Overview

UPMC Health Plan is looking forService Coordinator Supervisorpositions to join the Community HealthChoices team in our Philadelphia, PA region and surrounding areas. This is afull-timeposition that will require working from home and in office. Community HealthChoices (CHC) is Pennsylvania's managed care long-term services and supports (LTSS) program serving seniors and individuals with physical disabilities in the Commonwealth who are covered by Medicare and Medicaid. The role provides service coordination services according to the level of need. The Service Coordinator Supervisor will be responsible for the performance of General Service Coordination duties as well as specific responsibilities as assigned to each level of service.

Key Responsibilities

• Monitors and collects information related to staff and program performance.
• Conducts regularly scheduled team meetings to communicate key information.
• Oversees the operational functions of the LTSS Team, primarily focused on the service coordination model and the service coordinators.
• Recruits, trains, and orients new staff and ensures that training and education needs of staff are met.
• Manages the day-to-day service coordination functions including the approval of service plans and staff activity.
• Conducts onsite service coordination evaluations.
• Participates in the development and implementation of LTSS policies, goals, and objectives.
• Evaluates service coordination staff by selecting, scheduling, assigning, and assessing performance.
• Participates in relevant department and organization committees.
• Reviews performance against goals and makes recommendations for improvement.
• Participates in service coordination rounds, ICTs, and program development.
• Performs other duties as assigned.
