Job Description
Key Responsibilities
- Serve as a resource for participants in the program.
- Develop referral relationships with healthcare providers, social workers, and other community-based service providers.
- Perform home visits for the purpose of providing education, support, home health assessments, and obtaining and completing consents.
- Follow-up on provider referrals and assist families in addressing barriers to health care access.
- Schedule patient appointments.
- Demonstrate strong organizational and planning skills.
- Provide outreach to the community.
- Assist in the development of resource materials and in the preparation and maintenance of accurate records and data.
- Complete all reports to document activities and provide data to measure impact.
- Perform other administrative office duties.
- Maintain department orderliness and cleanliness.
- Perform other duties as assigned.
- Adhere to downtime procedures and established departmental policies, procedures, and objectives.
- Perform both essential and non‑essential job functions in a safe manner as identified by Humboldt Park Health.
- Demonstrate understanding and model the mission and core values of Humboldt Park Health.
- Enhance professional growth and development by accessing educational programs, job‑related literature, in‑service meetings, and workshops/seminars.
- Check hospital email daily.
- Perform any other related duties as assigned.
Required Qualifications
- Completion of Community Health Worker program required (complete within 1 year of hire).
- Ability to speak and write English to ensure safe and efficient job performance.

Preferred Qualifications
- Education (Preferred): High School or better.
- Licenses & Certifications (Preferred): CPR – Cardio Pulmonary Resuscitation.
- Language (Preferred): Ability to read and write Spanish.
- Behavior (Preferred): Enthusiastic – shows intense and eager enjoyment and interest.
- Motivation (Preferred): Self‑Starter – inspired to perform without outside help.
