Job Description

Position Overview

Oak Street Health – now a part of CVS Healthcare Delivery – is seeking a Medical Director, Population Health in Treehouse. This role is at the forefront of the value-based care movement, co-leading clinical and strategic initiatives across Population Health and Central Services, while maintaining a clinical practice (20–40% of time). You will work closely with directors, vice presidents, data engineering, and product teams to drive innovative programs that include AI technologies for value-based care, centralized telehealth services, specialty care, and more.

Key Responsibilities

  • Provide clinical leadership for Population Health programs, ensuring high-quality, patient-centered care while managing a 20–40% clinical practice.
  • Collaborate with directors and vice presidents across Population Health and Central Services to drive strategic clinical initiatives, including AI-driven care models, telehealth, and specialty services.
  • Act as the liaison with clinical and operational leaders to coordinate efforts, gather feedback, and ensure excellent communication across diverse teams.
  • Participate in organization-wide planning, strategy, and staffing, including vetting new opportunities and leading workgroups or committees.
  • Foster a culture of innovation, active listening, and accountability that embodies the “OAKY” values.

Required Qualifications

  • Must have US work authorization.
  • M.D., D.O., APN, or PA clinical training with an active license to practice.
  • Board certification in Internal Medicine, Family Medicine, Preventative Medicine, or Geriatrics.
  • Fellowship training, professional degrees (e.g., M.B.A., J.D., M.P.H.) or 2–5 years of management consulting experience.
  • Experience with managed care, value-based practices, and using metrics to drive clinical strategy.
  • Strong communication and effective collaboration skills with a diverse group of clinical and operational leaders.

Preferred Qualifications

  • Training and experience in providing primary care.
  • Prior clinical leadership or supervision experience.
  • Experience with payer-provider collaboration and developing new care processes.
  • A passion for driving clinical excellence and embodying “OAKY” values such as radiating positive energy and taking ownership.
